Coaching Agreement
Client Name: _____________________________________________
This agreement between Gayle Earle, here after referred to as Coach,
and the above-named client will begin on __________________and will
continue for a minimum of one month (4 weeks). The fee for the
initial meeting is $130.00. The monthly (4 week) fee is $320 payable
in advance each month. Individual sessions are $85 per hour.
The services to be provided by the Coach to the client are coaching
session in-person or tele-coaching, as designed jointly with the
client. Coaching, which is not advice, therapy, or counseling, may
address specific personal projects, business success, or general
conditions in the client’s life or profession. Other coaching
services include value clarification, brainstorming, identifying
plans of action, examining modes of operation in life, asking
clarifying questions, and making empowering requests.
Upon completion of the first month, coaching may continue on a
month-to-month basis or convert to per session basis. The client and
coach agree to provide one another with a 48 hour notice in the
event it is desired to cancel further services.
The coach promises the client that all information provided to the
coach will be kept strictly confidential.
Throughout the working relationship the coach will engage in direct
and personal conversations. The client can count on the coach to be
honest and straightforward in asking questions and making request.
The client understands that the power of the coaching relationship
can only be granted by the client – and the client agrees to do just
that: have the coaching relationship be powerful.
Our signatures on this agreement indicate full understanding of and
agreement with the information outlined above.
